Darkwor said it right - Pagans are heathens but not all heathens are pagans. You can be a "heathen" or "heretic" by opposing judeo-christianity and other organized religions, but that doesn't mean you're automatically pagan. In order to be pagan, like all other beliefs, one must practice it or follow it. There are certain bands - majority of them being German - which are 100% Pagan, with every member of the band participating it. From what I remember, that band Menhir had a hard time finding a new drummer because they wanted a drummer that followed and shared their beliefs.
That being said, Wicca is a form of Paganism, so would an all Wiccan metal band be considered Pagan metal? haha
